ALL PLAYERS MUST SUBMIT THE REQUIRED PAPERWORK TO ATTEND TRYOUTS

**  Niskayuna Soccer Club registrations are going online!  However, until we get started, you MUST complete the 2007 Tryout form below as well as submit the required documents to try-out!  Information regarding online registration will be sent in the late summer! **

 To tryout, please include the following:

1.   Completed and signed Tryout Form (pdf)

2.   Completed and signed Medical Release Form (pdf)

3.   2x2 inch player photograph (photo can be cut from group photo / face only)

4.   Copy of Birth Certificate if player is new to the club.

5.   Check for $195.00 made payable to Niskayuna Soccer Club.

U12, U14, U16-U19 Tryouts to be held in June of 2007

U12   8/1/95 – 7/31/97                 U14   8/1/93 – 7/31/95

U16   8/1/91 – 7/31/93                 U17   8/1/90 – 7/31/91

U18   8/1/89 – 7/31/90                 U19   8/1/88 – 7/31/89

U8 and U10 Teams will be determined in the fall

U8     8/1/99 – 7/31/01                  U10   8/1/97 – 7/31/99
